Kidney Health Australia's Operation Angel Award is a part of the National Awards Program which recognises the unsung heroes of the kidney sector – the carers, the patients, doctors and nurses and groups who work tirelessly to support those with kidney disease and to increase awareness of the need to reduce the burden of kidney disease.

HOW DO I NOMINATE MY ANGEL?
Kidney Health Australia's Operation Angel Awards are given to outstanding people who have made exceptional contributions to the kidney community.
Nominations are now open for the 2014 awards.
Download the guidelines and nomination form from the link below, then follow the instructions outlined to nominate your 'Angel'.
The nominee may be an individual, a family, or an organisation and will represent one of the following categories:
Nominations must be submitted by midnight on Saturday 31 May 2014
Submissions should include:
- Completed and signed nomination form
- Submission (maximum 250 words)
- Photo (less than 4Mb)
- Written reference (reference only required if you are nominating yourself)
